Considering the sensory dimension of clinical decision-making is a challenge for the training of health professionals. Therefore, how can we help them stimulate a form of sensory acuity? We conducted a study in partnership with the Simulation Laboratory of a Canadian hospital to understand the sensory work accomplished by nurses during a clinical decision-making. To do this, we analyzed the interactions occurring during focus groups to reveal the sensory work performed by nurses during the clinical evaluation of patients (personas).

The first “test-tube baby” in Japan was born in March, 1983 at Tohoku University Hospital. Since then ten years have passed. Table 1 indicares the clinical results of in vitro fertilization in this country. As it shows, more than 145 institutions perform IVF, and more than 3,000 babies have now been born using this procedure.According to the recommendations issued in October, 1983 by the Japan Society of Obstetrics and Gynecology, IVF is defined as a medical practice for treating infertility, and (...) this procedure is performed only on married couples de jure. Eggs and embryos are not donated for IVF in Japan, nor does any “lending” or “leasing” of wombs take place. However, inFebruary, 1988 the Japan Society of Obstetrics and Gynecology issued new recommendations which allow clinical use of frozen embryos and eggs. The first baby from a frozen embryo was born in 1989, and 60 babies have been born to date. (shrink)

In contemporary onomastics the usage of the full name – given name and surname – lends a certain formality or seriousness to an utterance. It is often assumed that such pragmatics in the employment of names may be easily transferred to the ancient world, but we should none the less confirm our assumptions with textual evidence. This paper will present evidence from the letters between Marcus Aurelius and Marcus Cornelius Fronto which demonstrates not only that the fuller Roman name was (...) used for formal occasions, but also that it could, as in the contemporary world, be employed in an ironic, mock-formal manner to intimate rather the opposite of formality, friendship. This paper seeks to bridge two aspects of Fronto's letters, erotics and rhetoric, by demonstrating that Fronto himself merges the two areas in his discourse with Marcus Aurelius about their relationship. Whereas some letters suggest an unequal relationship based on power, others encourage the identification of Fronto with Marcus. Fronto achieves this identification by structuring their relationship itself as a metaphor in which he and Marcus are equated and linked by epistolary bonds. I close by discussing why the epistolary genre (...) in particular is an apt site for the merger of metaphor and love. (shrink)

The process learning for adults training joins in a space/time which can be considered as « communicative action ». Learning is for us a part of a space of sense referred to the notion of passage and thus process. It is well known today that one of its modalities, for adults as for children, leans on the imitation. On the other hand, the game of the interactions makes that the learnings are not passed on without an autonomous and mediate renegotiation (...) of the subjects (vicariant learning). This relation between the partners will base itself on characteristics which allow to understand the interpretation as a modality, an essential medium to an «emancipator learning». On that basis, the constructed knowledge, the knowledge in elaboration, engages a conversion of the look in the training for adults, but also that of the learner towards an empowerment of a subject and his emancipation. A challenge in human genome research is how to describe the populations being studied. The use of improper and/or imprecise terms has the potential to both generate and reinforce prejudices and to diminish the clinical value of the research. The issue of population descriptors has not attracted enough academic attention outside North America and Europe. In January 2012, we held a two-day workshop, the first of its kind in Japan, to engage in interdisciplinary dialogue between scholars in the humanities, social (...) sciences, medical sciences, and genetics to begin an ongoing discussion of the social and ethical issues associated with population descriptors. In the following pages we discuss three historical cases of moral economies in science: Drosophila genetics, late twentieth century American astronomy, and collaborations between American drug companies and medical scientists in the interwar years. An examination of the most striking differences and similarities between these examples, and the conflicts internal to them, reveals constitutive features of moral economies, and the ways in which they are formed, negotiated, and altered. We critically evaluate these three examples through the filters of rational choice, (...) utility, and American pragmatism, using the latter to support the conclusion that there is no single vision of moral economies in science and no single theory—moral, political, social—that will explain them. These filters may not be the only means through which to evaluate the moral economies examined, but aspects of each appear prominent in all three cases. In addition, explanations for decisions are often given in the language of these theories, both at the macro (policy) level and at the local level of the moral economies we discuss. In light of such factors, the use of these frameworks seems justified. We begin with an attempt to define the nature of moral economies, then move to a consideration of scientific communities as moral communities operating within material and other constraints which we relate to wider questions of political economy and societal accountabilities. The debate over whether free will and determinism are compatible is controversial, and produces wide scholarly discussion. This paper argues that recent studies in experimental philosophy suggest that people are in fact “natural compatibilists”. To support this claim, it surveys the experimental literature bearing directly or indirectly upon this issue, before pointing to three possible limitations of this claim. However, notwithstanding these limitations, the investigation concludes that the existing empirical evidence seems to support the view that most people have compatibilist (...) intuitions. (shrink)

The following is a transcript of the interview I (Yasuko Kitano) conducted with Neil Levy (The Centre for Applied Philosophy and Public Ethics, CAPPE) on the 23rd in July 2009, while he was in Tokyo to give a series of lectures on neuroethics at The University of Tokyo Center for Philosophy. I edited his words for publication with his approval.

In a refrigerated warehouse, workers organize distribution and exchange of frozen seafood by the spatial and temporal arrangement of loads. Using videotapes of workers' activities and interviews, this paper investigates how workers organize space, time and artifacts in the activity of frozen seafood distribution and exchange, and how organized space, time and artifacts systematize workers' multiple courses of actions and give direction to them. Particular attention is paid to how the workers use artifacts such as various documents and computers as (...) part of their practices, because the activity of distribution and exchange in this workplace is regarded as the activity of organizing space and time with various artifacts in multiple contexts. Through the analysis of situated literacy of using artifacts, it was shown that the activity of using artifacts is always embedded in multiple contexts. At the same time, diverse artifacts organize multiple contexts of work. Thus, there is the reflexive interaction between the artifacts and the contexts. (shrink)

This study aimed to: (1) develop and evaluate the Moral Distress Scale for Psychiatric nurses (MDS-P); (2) use the MDS-P to examine the moral distress experienced by Japanese psychiatric nurses; and (3) explore the correlation between moral distress and burnout. A questionnaire on the intensity and frequency of moral distress items (the MDS-P: 15 items grouped into three factors), a burnout scale (Maslach Burnout Inventory — General Survey) and demographic questions were administered to 391 Japanese psychiatric nurses in 2007—2008. These (...) nurses experienced relatively low levels of moral distress despite the fact that they were commonly confronted by morally distressing situations. All the circumstances in which the participants experienced moral distress were included in the ‘low staffing’ factor, which reflects the characteristics of Japanese psychiatric care. The frequency score of the low staffing factor was a significant predictor of burnout. (shrink)

Substantial evidence implicates fast axonal transport (FAT) defects in neurodegeneration. In Alzheimer's disease (AD), it is controversial whether transport defects cause or arise from amyloid‐β (Aβ)‐induced toxicity. Using a novel, unbiased genetic screen, Morihara et al. identified kinesin light chain‐1 splice variant E (KLC1vE) as a modifier of Aβ accumulation. Here, we propose three mechanisms to explain this causal role. First, KLC1vE reduces APP transport, leading to Aβ accumulation. Second, reduced transport of APP by KLC1vE triggers an ER stress (...) response that activates the amyloidogenic pathway. Third, KLC1vE impairs transport of other KLC1 cargos that regulate amyloidogenesis, promoting Aβ retention within the secretory pathway. Collectively, KLC1vE perpetuates a vicious cycle of Aβ generation, kinase dysregulation, and global FAT impairment that inevitably leads to cellular toxicity. These concepts implicate alternative splicing of KLC1 in AD and suggest that the reciprocal influence of transport mechanisms on disease states contributes to neurodegeneration. (shrink)

Ensuite, une réflexion sur la bio-région unifiée autour d’une vallée, d’une plaine ou d’un bassin permettant de transcender des limites administratives arbitraires. (...) Enfin, le modèle de la « ville horizontale », qui s’est concrétisé par le projet de Parc bleu autour des structures hydrographiques et une urbanisation continue qui s’étend de la Belgique à la Ruhr et la Randstad. Three experimental tasks—spontaneous telling of a story, reading, and parsing the story—were used to determine whether empirical data reflect the narrative structure of stories and can be predicted by a plot unit analysis of the stories (Lehnert, 1981). It was found that spontaneous pause durations at sentence breaks were highly correlated with the importance of these breaks as predicted theoretically. Only low correlations were obtained, however, when reading pause durations were correlated with the model. As for parsing values, the value (...) of the correlation coefficients depended on whether stories had sufficient superficial linguistic cues to help the subjects in parsing. It was concluded that spontaneous pausing not only reflects the narrative structure of stories, but can be used as a guide to constructing theories of narrative structure as well as for deciding between competing theories. Telemedicine changes clinical practice and introduces new ways of distributing tasks between physicians and nurses, and particularly the delegation of sensory assessments during remote physical examinations. As nurses become more involved in patient assessment and clinical decision‐making, the quality of physician–nurse collaboration has been recognized as essential to ensure quality patient care. However, few studies have examined physician–nurse interactions during teleconsultations. This article presents the results of an empirical study of nurse–physician communication during remote physical examinations. In partnership with a (...) university‐affiliated hospital in Ontario, Canada, we observed and recorded 10 simulated postsurgical consultations in orthopedics (involving a physician, a patient, and an on‐site nurse) and conducted auto‐confrontation interviews with physicians. The results of the thematic analysis of the interviews informed the selection of consultation sequences for in‐depth interaction analysis. The findings demonstrate the nurse's essential role during remote physical examinations and reveal specific practices accomplished by the nurse to ensure successful nurse–physician collaboration. The interview data shows how physicians view the nurse's role and contributions. The findings contribute to our understanding of the collaborative nature of sensory assessments during remote physical examinations in telemedicine and can inform the development of training programs for professionals focusing on communication skills. Le terme hébreu qui désigne à la base les « cornes » semble également pouvoir décrire un phénomène lumineux dans deux textes de la Bible hébraïque : Habacuc 3 et Exode 34. Le présent article propose, premièrement, d’analyser l’usage polysémique de qrn en Ha 3 en étudiant le rôle de cette racine comme attribut divin dans la Bible hébraïque ainsi que dans le Proche-Orient (...) ancien. Deuxièmement, il replace l’utilisation polysémique de qrn dans le cadre socio-historique de rédaction du texte d’Ha 3. L’article propose enfin d’associer cette lecture à l’iconographie royale séleucide, qui utilise notamment les cornes rayonnantes dans leur propagande impériale, comme attesté dans la numismatique. The Networks of Centres of Excellence programme is perhaps Canada's most dramatic science policy innovation since theFirst World War. This article traces its development, using documentary analysis and interviews with the policy actors responsible for conceiving and implementing the programme.Established in 1989, the networks were explicitly designed to change the norms of science. The intention was to instil an approach to long-term fundamental research that considered possibilities of use from the start. This paper explores the interface between users and producers of translational science through three case studies. It argues that effective TS requires a breakdown between user and producer roles: users become producers and producers become users. In making this claim, we challenge conventional understandings of TS as well as linear models of innovation. Policy-makers and funders increasingly expect TS and its associated socioeconomic benefits to occur when funding scientific research. We argue that a better understanding of the hybridity between users (...) and producers in TS is essential to encouraging effective TS activities. In arguing for broader understandings of the hybrid roles of user/producers in TS we rely on empirical observations made during our four-year study of three translational pathways here labeled clinical, commercial, and civic.
